Position Purpose:
The Registered Nurse acts as the Clinical Liaison responsible for supervising all client care activities to assure compliance with current standards of accepted nursing and medical practice, including supervising, training, and orientation processes with care teams and the overall direction of home care services.
This person will establish, implement, and evaluate goals and objectives for home care services that meet and promote the standards of quality and contribute to the total organization’s philosophy.
ESSENTIAL FUNCTIONS
Performs client assessment upon admission, change of condition, and as required by state law.
Develops the plan of care and makes necessary revisions as client status and needs change.
Collaborates with, counsels, and educates the client and family to develop a plan of care to meet specific client needs.
Provides direct client care as defined in the State Nurse Practice Act.
Identifies discharge planning needs as part of the care plan development and implements prior to discharge of the client.
Performs skills competency assessments and participates in initial and on-going training of care staff.
Performs supervisory visits to instruct, supervise, and evaluate personal care staff as required by state law.
Collaborates with client care team to implement plan of care.
Communicates with client’s representative regarding the client’s needs and reports any changes in the client’s condition; obtains/receives physician’s orders as required.
Communicates with community health related persons to coordinate the care plan.
Participates in on-call duties as defined by the on-call policy.
Assists with quality assurance process improvement program in conjunction with Branch Director.
Participates in audits with licensing and regulatory bodies.
Acts as Brand Ambassador for Right at Home at community related functions.
EDUCATION / SKILLS / ABILITIES / AVAILABILITY
Graduate of an accredited school of nursing.
Current licensure in the state and CPR certification.
One (1) year of home care or home health care experience preferred.
Experience in a healthcare training setting a plus (CPR, AED, CNA, etc).
Licensed driver with automobile that is in good working order and insured in accordance with the organization requirements.
Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; nursing skills per competency checklist.
Ability to relate to clients and care staff in a way that is sensitive to the unique needs of the individual.
Competent computer skills and a familiarity with electronic medical records.
Ability to work in a discrete manner maintaining confidentiality of sensitive information; self-starter with ability to multi-task in a fast paced environment; excellent organizational skills and ability to prioritize work load and adhere to strict deadlines.
