Sentara - Oncology Service Line is currently hiring a, Palliative Care - Oncology Nurse Navigator (RN).
This is a Full-Time position with day shift hours and great benefits! An Oncology Nurse Navigator (RN) supports cancer patients.
Responsible for providing patient and family education regarding findings, diagnosis, workup, lifestyle recommendations, and treatment plans.
Assessing psychosocial needs and assisting with barrier resolution.
Responsible for approximately 250 patients per year.
This position is integral in improved patient satisfaction, staff satisfaction, provider satisfaction, reduced length of stay, reduced ED visits and readmissions, and quaternary growth.
The primary location will be at Sentara Brock Cancer Center with ability to travel if needed to meet patient, service line, and palliative care needs in the Hampton Roads region, including Northeastern North Carolina.
The role will be supporting cancer patients who are appropriate for and established with palliative care.
Primary duties include but are not limited to: Facilitating communication between patient, caregiver, and care team regarding symptom management Facilitating discussions on goals of care, resuscitation status, exploring treatment options (i.
e.
PEG tubes), dealing with unpredictable prognosis, end of life issues/advance care planning, withdrawal of life prolonging treatments, need for hospice / home health services, and family conferences.
Assessing client needs and barriers to care through the continuum of care.
Providing support, education on medications, treatments, and resources available internally within Sentara and externally in the community and nation.
Facilitating referrals to appropriate resources.
Leading and participating in various quality initiatives related to Palliative Care and Oncology Service Line.

Minimum Requirements *Experience: 3+ years of clinical nursing experience required.
*Education: RN-Associate's Degree OR RN-Bachelor's Level Degree OR RN-Diploma (Nondegree) OR RN-Master's Level Degree *BLS required within 90 days of hire and maintained thereafter (if in clinical setting) Specialty certification based on specialty area required within one year of eligibility (OCN, CBCN) *Prior work experience with oncology patients, palliative care, hospice care, and/or case management is strongly preferred.
